Direct answer: A picosecond laser is not a default treatment for every raised scar. Hypertrophic scars and keloids behave differently from depressed acne scars, and additional injury can worsen some scars. A clinician should first confirm the diagnosis, age, activity, symptoms, body site, skin tone, keloid tendency, and prior response. Treatment may instead involve observation, silicone, pressure, medication injections, vascular devices, surgery, or combinations.

Before choosing a faster pulse, decide whether the scar is still trying to grow.

Why is diagnosis essential?

A hypertrophic scar stays within the original wound boundaries, while a keloid can extend beyond them. Other skin growths can also be mistaken for scars.

The scar’s color, firmness, itch, pain, growth pattern, and age help determine whether pigment, blood vessels, collagen, tension, or inflammation is the principal target.

What does picosecond evidence show?

The 2026 systematic review reports promising findings across scar types, but the studies vary and acne-scar evidence is more developed than a universal protocol for raised scars.

A laser can sometimes address color or texture as part of a broader plan without being the right tool for scar bulk or growth behavior.

What should you do next?

Ask for the exact diagnosis and endpoint. If treatment is proposed, discuss recurrence, pigment risk, settings, test spots, sun protection, follow-up, and alternatives.

For an individualized decision, use Esthetics Intelligence. In Alpharetta and North Atlanta, I also consider ultraviolet exposure, climate, timing, and reliable follow-up.

When should you seek medical care?

Seek assessment for a rapidly growing, painful, ulcerated, bleeding, or newly changing lesion, or for infection signs. Do not assume every raised lesion is a benign scar.

This is where a clear diagnosis and documented product or procedure history are more useful than another round of online guessing.

Frequently asked questions

Can laser make a raised scar worse?

Yes. Additional inflammation or injury can worsen some scars, particularly in keloid-prone patients.

Is a keloid the same as a hypertrophic scar?

No. Their growth patterns and recurrence behavior differ.

Can a picosecond laser flatten a keloid?

It should not be promised as a stand-alone universal flattening treatment; diagnosis and combination planning matter.
