I have had some version of every one of these conversations in the exam room over the past several months, and I want to start this piece the way I try to start those conversations: by being honest about what NAD+ actually is, what the evidence for topical use currently does and does not support, and why the way this ingredient is being marketed right now is running meaningfully ahead of the research behind it. That gap between marketing and evidence is not unique to NAD+ — I have watched it happen with collagen supplements, with several "miracle" retinoid alternatives, and with more than a few ingredients that quietly disappeared from shelves once the initial excitement faded. But I think NAD+ deserves a more careful look than most, because the underlying biology it's built on is genuinely interesting and well-established at the cellular level. The question isn't whether NAD+ matters to your cells. It clearly does. The question is whether putting it on your skin in a jar does anything close to what that cellular biology would suggest, and that is a much narrower, much less settled question than the marketing implies.


I also want to be transparent about why I'm writing this now. NAD+ has crossed a threshold this year that a lot of trending ingredients never reach — it has gone from a niche supplement marketed to biohackers and longevity enthusiasts to a mainstream skincare ingredient appearing in national retail products, with search interest climbing sharply. That crossover moment is exactly when I think a physician-led voice matters most, because it's the moment when the loudest marketing claims reach the widest audience, often well before the clinical research has caught up to validate or correct them.


What NAD+ Actually Is, Biologically


NAD+, or nicotinamide adenine dinucleotide, is a coenzyme present in every living cell in your body. It is not a new discovery — the molecule has been studied in basic cell biology for over a century — but the specific interest in NAD+ as a longevity and anti-aging target is more recent, driven largely by research into how NAD+ levels change as we age and what role the molecule plays in cellular energy production and DNA repair.


Functionally, NAD+ exists in your cells in two related forms, oxidized (NAD+) and reduced (NADH), and the two constantly interconvert as part of the electron transport chain — the core biochemical process your mitochondria use to convert nutrients into usable cellular energy in the form of ATP. Without adequate NAD+, this energy-production process slows, which is part of why NAD+ is often described in shorthand as "cellular energy currency." That description isn't wrong, but it is a significant simplification of a much more layered biological role.


Beyond energy metabolism, NAD+ is also a required substrate for a family of enzymes called sirtuins, which are involved in regulating gene expression, DNA repair, and cellular stress responses — some of the same pathways implicated in caloric restriction's effects on longevity in animal models. NAD+ additionally serves as a substrate for PARP enzymes, which play a central role in repairing DNA damage, including the kind of damage caused by ultraviolet exposure, a genuinely relevant connection for skin biology specifically.


Here is the piece of biology that gets lost in most of the marketing: NAD+ levels in tissue, including skin, do measurably decline with age. This is well documented in the research literature and is one of the more consistent findings in aging biology broadly. That decline is real, and it plausibly contributes to reduced cellular resilience, slower DNA repair, and reduced energy availability for the active, energy-expensive processes your skin cells carry out constantly — collagen synthesis, barrier repair, and the turnover of the outer skin layer among them.


So the foundational premise behind NAD+ skincare — that a molecule your cells need more of as you age, and that measurably declines in tissue over time, might be beneficial to support — is not manufactured hype. It is grounded in real, reasonably well-established cell biology. Where the science gets much thinner, and where I think patients deserve real clarity, is in the leap from "this molecule matters enormously inside your cells" to "applying it topically, on the outside of your skin, meaningfully restores that decline."


The Oral-Versus-Topical Distinction Almost Nobody Explains Clearly


This is, in my experience, the single most confusing part of the entire NAD+ conversation for patients, and it's rarely addressed directly by either supplement or skincare marketing.


NAD+ itself is a relatively large, charged molecule. Large, polar molecules generally do not cross the skin barrier efficiently on their own, which is one of the fundamental principles of topical formulation and part of why so much cosmetic chemistry effort goes into either modifying molecules to improve penetration or delivering them via alternative mechanisms like liposomal encapsulation or microneedling-assisted delivery. This is the same basic barrier-permeability challenge that has shaped decades of skincare formulation for other actives — it's why, for example, plain vitamin C in its most stable, effective form requires a specific pH and concentration range to penetrate meaningfully, and why hyaluronic acid's molecular weight determines whether it sits on the surface or penetrates more deeply.


For oral NAD+ supplementation, the relevant question is different and somewhat better studied. Oral NAD+ itself is largely broken down during digestion before it reaches systemic circulation intact, which is why most oral longevity supplements don't actually supply NAD+ directly. Instead, they typically supply precursor molecules — nicotinamide mononucleotide (NMN) or nicotinamide riboside (NR) — that your body can convert into NAD+ through its own internal metabolic pathways after absorption. There is a meaningfully larger body of published human research on these oral precursors, including several placebo-controlled trials, examining effects on blood NAD+ levels, markers of metabolic health, and measures like insulin sensitivity and exercise capacity in specific populations, though even this research remains an active, evolving area rather than a settled one, and results have been mixed across different study populations and precursor forms.


For topical NAD+ skincare, the research picture is considerably thinner. Most topical NAD+ and NAD+-precursor products on the market today are formulated with the precursor molecules — commonly nicotinamide (a well-established, genuinely effective topical ingredient in its own right, more familiarly known as niacinamide) or, more recently, NMN specifically marketed for topical use. The clinical evidence specifically supporting topical NMN or topical NAD+ as delivering a measurable increase in skin cell NAD+ levels, let alone a measurable downstream improvement in wrinkle depth, elasticity, or other visible aging markers, is currently limited to early-stage research — small studies, in vitro cell culture work, and mechanism-focused research rather than the kind of larger, longer-duration, placebo-controlled clinical trials that back an ingredient like retinol or, again, niacinamide itself.


I want to be precise here rather than dismissive, because this distinction matters. "Limited evidence" is not the same as "no evidence" or "doesn't work." It means the research that would let a physician say, with real confidence, "topical NAD+ produces X percent improvement in Y measurable outcome over Z weeks" simply doesn't yet exist in the way it does for a small number of thoroughly studied actives. That's a meaningfully different, more honest statement than either "this is proven" or "this is a scam," and I think patients deserve that more precise middle ground rather than either extreme.


What Niacinamide Already Tells Us, and Why That Matters for the NAD+ Conversation


Here is something I think gets genuinely obscured in the current NAD+ marketing wave, and it's worth stating plainly: niacinamide, which is a form of vitamin B3 and a direct precursor in the NAD+ synthesis pathway, is one of the most thoroughly studied, well-evidenced topical actives in all of skincare. It has been the subject of numerous published, peer-reviewed clinical trials demonstrating measurable improvements in skin barrier function, reduction in hyperpigmentation, improved skin tone evenness, reduced fine lines, and improved sebum regulation, generally at concentrations in the 2 to 5 percent range, applied topically over eight to twelve weeks.


Niacinamide almost certainly exerts at least some of these benefits, at least in part, through supporting cellular NAD+ synthesis, since it is a direct metabolic precursor. In that sense, a well-formulated niacinamide serum has always been, functionally, an "NAD+ support" product — it just wasn't marketed using that specific molecular framing until the broader NAD+ longevity conversation created consumer demand for it.


I raise this not to suggest that newer NAD+-branded products are simply niacinamide relabeled — some genuinely do use different precursor molecules or delivery technologies, and the formulation landscape is evolving quickly. I raise it because I think it's a useful, honest anchor point for the conversation: if a product's actual NAD+-supporting mechanism runs through niacinamide, you are looking at a well-evidenced ingredient wearing newer branding, and that's a meaningfully different situation than a product built around topical NMN or topical NAD+ itself, where the direct clinical evidence is considerably younger and thinner. Reading the actual ingredient list, not just the marketing headline on the front of the package, matters enormously here.


What the Marketing Claims, and Where That Outpaces What We Can Currently Support


I want to walk through the specific claims I'm seeing in NAD+ skincare marketing right now, because I think patients benefit from seeing them held up individually against what the current evidence actually supports.


"NAD+ skincare restores your skin's cellular energy." This claim rests on real biology — NAD+ genuinely is central to cellular energy metabolism, and levels genuinely do decline with tissue age. What it does not currently have behind it is direct clinical evidence that a topical product meaningfully restores measurable cellular NAD+ levels in human skin, at a concentration and delivery mechanism achievable in a consumer product, in a way that translates to a visible outcome. The mechanism is plausible. The specific topical claim, as currently marketed, is ahead of the confirming data.


"NAD+ activates longevity genes in your skin." This is a reference to sirtuin activation, and it is, again, grounded in real cell biology — NAD+ is a required cofactor for sirtuin enzyme function. But the sirtuin-activation research that gets cited in this context comes overwhelmingly from cell culture studies, animal models, and research on systemic, whole-body NAD+ levels — not from studies demonstrating that topical skin application produces meaningful local sirtuin activation in human skin specifically. This is one of the more significant gaps between the underlying science, which is genuinely exciting, and the specific topical skincare claim being built on top of it.


"NAD+ is the next retinol." I understand why this comparison is appealing from a marketing standpoint, but I think it actually does a disservice to how skincare evidence is supposed to work. Retinol's efficacy is supported by decades of published clinical trials, a well-characterized mechanism of action at the cellular receptor level, and consistent, reproducible results across multiple independent research groups. NAD+ skincare, at this point, has none of that depth of evidence behind it yet. It may, over time, develop a genuinely strong evidence base — the underlying biology gives it a real, plausible path to do so, unlike some ingredients that never had a coherent mechanism to begin with. But "may develop strong evidence over time" and "is the next retinol" are very different claims, and I think patients deserve to know which one is currently true.


What I Actually Recommend to Patients Asking About NAD+ Right Now


I don't think the honest, evidence-transparent answer to "should I use NAD+ skincare" is a flat no, and I also don't think it's an enthusiastic yes. It's something closer to: it's reasonable to try, at a modest cost, with realistic expectations, and without abandoning the ingredients that already have strong evidence behind them.


If a patient is specifically interested in supporting their skin's NAD+ pathway with strong existing clinical support, I point them first to niacinamide — a well-evidenced, inexpensive, widely available active that already works through this exact mechanism and has decades of research behind its skin benefits, independent of how it's currently being marketed.


If a patient wants to try a specifically NAD+ or NMN-branded topical product because the newer formulation technology or delivery mechanism genuinely interests them, or because they've had success with related actives and want to experiment, I don't discourage that — skincare experimentation within a reasonable budget, layered onto an existing evidence-based routine rather than replacing it, is a low-risk way to explore an emerging ingredient. What I do encourage is setting realistic expectations: this is an ingredient with promising underlying biology and thin current topical-specific clinical evidence, not a proven, dramatic anti-aging breakthrough, and any visible improvement should be evaluated honestly over several months rather than assumed in advance.


I actively discourage patients from discontinuing well-evidenced parts of their existing routine — a broad-spectrum sunscreen, a retinoid, a proven exfoliating acid, a well-formulated moisturizer — to make room for an NAD+ product on the assumption that it's a superior replacement. At this point in the evidence timeline, that would mean trading something with strong data behind it for something with considerably less, and that's a trade I don't think is currently justified by what we actually know.


For patients already taking an oral NAD+ precursor supplement for general health or longevity reasons and wondering whether a topical product is redundant or complementary: based on current understanding of skin barrier permeability, oral supplementation and topical application are likely addressing different physiological compartments rather than duplicating each other's effect. Oral precursors, once absorbed, contribute to systemic and cellular NAD+ pools throughout the body, including skin cells via normal circulation and cellular uptake. Topical application, if it works as intended, would be attempting a more localized, direct effect at the skin surface and upper layers. Whether the topical route adds a meaningful benefit on top of adequate systemic levels achieved through oral supplementation or a healthy baseline diet is not yet clearly established either way.


Why I Think This Ingredient Is Worth Watching, Not Dismissing


I want to be clear that my caution here is about the current specific evidence for topical application, not about the underlying scientific interest in NAD+ as a target for skin and cellular aging more broadly. The connection between declining cellular NAD+ levels and multiple markers of tissue aging is one of the more consistently reproduced findings in aging biology across different tissue types, and it would not surprise me at all if, over the next several years, better-designed clinical trials — larger sample sizes, longer duration, placebo-controlled, using validated measurement tools for wrinkle depth and elasticity rather than just self-reported satisfaction — establish a genuinely well-supported topical NAD+ or NAD+-precursor protocol with real, quantified benefit.


I also think it's entirely possible that some of the current formulation approaches will turn out to be more effective than others, once we better understand which precursor molecules, which concentrations, and which delivery technologies actually achieve meaningful cellular uptake through topical application — the same kind of formulation refinement process that eventually produced today's genuinely effective, well-tolerated retinoid and vitamin C formulations after years of less effective early versions.


What I don't think is fair, to patients or to the ingredient's genuine scientific potential, is marketing language that borrows the weight and credibility of decades of basic NAD+ cell biology research to imply a level of topical-specific clinical validation that doesn't yet exist. That gap is exactly the kind of thing a physician-led, evidence-transparent voice should be naming clearly, rather than either amplifying the hype or dismissing a genuinely interesting area of research out of excessive caution.


The Broader Pattern This Fits Into


I've watched this specific pattern play out with several ingredients now over the years I've practiced: a molecule with genuinely interesting, well-established basic science gets identified as a promising target, oral supplementation research develops first and reaches a reasonable evidence threshold, and then skincare marketing moves quickly to apply the same language and the same excitement to a topical product before the topical-specific research has caught up. Collagen supplements and collagen-branded topicals followed a similar arc. So, to a lesser degree, did several probiotic and postbiotic skincare launches.


Sometimes that gap closes, and the ingredient earns its place as a genuinely evidence-backed topical active — niacinamide itself is arguably the best example of an ingredient that, decades ago, made exactly this kind of leap from internal metabolic relevance to well-proven topical benefit, and the clinical evidence eventually caught up to validate it thoroughly. Sometimes the gap doesn't close, and the topical version quietly fades from shelves once the initial marketing cycle ends, regardless of how compelling the internal cell biology genuinely was.


I don't know yet which path topical NAD+ specifically will take, and I think any physician claiming certainty either way, in either direction, is overstating what we currently know. What I can tell you is what the evidence currently supports, what it doesn't yet support, and what a reasonable, low-risk way to explore the ingredient looks like if you're curious — which is the standard I try to hold every ingredient conversation to in this practice, whether the ingredient is a decades-old, thoroughly proven active or a genuinely exciting, still-developing one like this.


A Note on Longevity Claims Generally, Because This Connects to a Larger Conversation


I want to close with something slightly broader than NAD+ specifically, because I think it's relevant to how patients should evaluate the growing number of "longevity" and "cellular health" claims appearing across skincare, supplements, and even some medical spa service menus right now. As interest in longevity medicine has grown rapidly — and genuinely, some of that growth reflects real, exciting advances in our understanding of aging biology — the pace of consumer-facing marketing claims has, in a number of cases, outpaced the pace of rigorous clinical validation behind them.


I think that gap deserves more explicit acknowledgment from providers and brands than it currently receives, and it's part of why I try to be specific, in every piece I write and every consultation I have, about which claims rest on strong evidence, which rest on plausible but unconfirmed mechanisms, and which are, honestly, further ahead of the science than the marketing suggests. I'd rather a patient trust me because I've been consistently honest about that distinction over years of care than because I told them what was most exciting to hear in a single visit.
