Direct answer: Nanofat is not the same as structural facial fat transfer. Both begin with a patient’s own fat, but nanofat is mechanically processed into a fine emulsion and used mainly for tissue-quality goals rather than reliable volume restoration. Structural fat grafting preserves larger fat parcels to add shape and volume. They differ in processing, injection technique, intended endpoint, predictability, and how results should be evaluated.

Two procedures can start with the same tissue and end with very different jobs.

What does processing change?

Nanofat processing disrupts mature adipocytes and creates an injectable emulsion associated with stromal and signaling components. Structural grafting aims to preserve viable fat parcels that can integrate and retain volume.

That is why nanofat should not be sold as a precise substitute for filler or as a volume guarantee.

How do the goals differ?

Nanofat studies focus on texture, pigmentation, scars, photodamage, and tissue quality. Structural grafting focuses more directly on contour and volume restoration.

Some treatment plans combine different fat fractions, but combination does not erase the separate risks, harvest requirements, and outcome variability.

What should you do next?

Ask which fraction is being used, how it is processed, the target layer, the intended endpoint, donor-site plan, recovery, and what the clinician will do if the response is uneven.

For an individualized decision, use Esthetics Intelligence. In Alpharetta and North Atlanta, I also consider ultraviolet exposure, climate, timing, and reliable follow-up.

When should you seek medical care?

Contact the treating clinician for escalating pain, blanching, visual symptoms, infection signs, severe swelling, or any unexpected neurologic or vascular symptom after an injection procedure.

This is where a clear diagnosis and documented product or procedure history are more useful than another round of online guessing.

Frequently asked questions

Does nanofat add volume?

It is not designed for predictable structural volume in the way larger-particle fat grafting is.

Do both require fat harvesting?

Yes. Both require an autologous donor procedure.

Is one always more natural?

No. Natural-looking outcomes depend on diagnosis, proportion, technique, restraint, healing, and the endpoint—not the label alone.
