I have had the same conversation more than a dozen times in the last two months.


A patient sits down for a consultation and says, "I saw something on TikTok about salmon DNA injections. People in Korea and the UK are getting them. Can I get that here?"


My answer is always honest, and it is always more complicated than a yes or no. Because the science behind what they are describing is genuinely interesting, and in some forms, genuinely well-supported. But the specific thing they saw on their phone — an injectable made from salmon DNA fragments — is not currently approved for use in the United States. And the gap between what is trending and what is legally and clinically available here is exactly the kind of gap that creates confusion, misinformation, and, occasionally, patients seeking out unregulated providers who are willing to inject something they should not be injecting.


I want to walk through this carefully, the way I do with patients in my office in Alpharetta. Not to dismiss the trend — there is real biology here worth understanding — but to separate what is established, what is promising but unproven, and what is simply not legally available to you right now, regardless of what an Instagram ad or a med spa in another country is offering.


What Polynucleotides Actually Are


Polynucleotides, often abbreviated PN, are long chains of nucleotides — the basic building blocks of DNA — extracted and purified from biological sources. They are not yet a treatment with the depth of long-term, large-scale clinical evidence that some of the older, more established categories in aesthetic medicine have accumulated. Promising and proven are not the same word, and a responsible practice should never blur that line for you.


The Part Nobody Wants to Say Clearly: The U.S. Legal Status


This is the section I consider the most clinically important part of this entire conversation, because it is the part most likely to actually protect you from harm.


As of now, injectable polynucleotide and PDRN products — the category that includes well-known international brand names that circulate heavily on social media — do not have FDA approval for injection in the United States. This is true regardless of how widely they are used in South Korea, the United Kingdom, parts of Europe, or other markets with different regulatory pathways. A product being legally injectable in Seoul or London does not make it legally injectable in Atlanta, and any provider telling you otherwise is giving you inaccurate information.


There is one important and specific exception worth knowing: a topical polynucleotide product has received FDA clearance in the United States, but specifically for topical application, most commonly delivered via microneedling, not via injection. This distinction is not a technicality. It is the entire difference between a compliant, regulated treatment delivered by a licensed provider using an approved delivery method, and an off-label or outright illegal injection using a product that has not gone through the FDA's safety and efficacy review process for that use.


I say this directly because I think patients deserve directness: if a provider in the United States is offering you an injectable "salmon DNA" or "PDRN" treatment, I would ask them, specifically, what product they are using, what its FDA status is for injectable use, and where they sourced it. Vague or evasive answers to that question are a meaningful red flag. Unapproved injectable products carry real risks — including unknown sterility standards, unknown purification consistency, unknown dosing accuracy, and the absence of the adverse event reporting infrastructure that comes with FDA-regulated products. This is not bureaucratic caution for its own sake. It is the actual mechanism by which patients are protected from contaminated, mislabeled, or inconsistently manufactured biologic products.

Understanding why this became such a significant social media moment helps explain both the appeal and the risk.


The trend has clear roots in South Korea, where polynucleotide and PDRN treatments have an established place in the aesthetic medicine landscape and a regulatory pathway that differs from the FDA process. K-beauty content has long had outsized influence on global skincare trends, and as regenerative aesthetics — treatments that work by stimulating the body's own repair processes rather than simply filling or relaxing — became the dominant philosophical shift in the aesthetics industry, PDRN and PN were a natural next chapter in that story, following closely behind the rise of exosome-based treatments, which I have written about separately because the two categories get confused with each other constantly despite being biologically distinct.


Social platforms amplified it further because the visual narrative is compelling: a treatment derived from fish DNA, marketed with phrases like "glass skin" and "second skin," delivered via a quick in-office procedure, with before-and-after content that performs well in short-form video. The ingredient story itself — DNA from salmon, repairing human skin — has the kind of counterintuitive, slightly startling quality that drives shares and comments, independent of the underlying clinical reality.


None of that makes the underlying science illegitimate. But it does mean that the volume and intensity of consumer interest significantly outpaced the regulatory and clinical infrastructure required to deliver it safely and legally in every market, including ours. That mismatch is precisely the situation that creates space for unregulated providers, medical tourism risk, and patients making decisions based on incomplete information.


What You Can Actually, Legally Access Right Now


This is the question my patients actually want answered, so let me answer it plainly.


Topical PDRN-containing formulations, applied either as a standalone topical or — more effectively — delivered via microneedling, represent the compliant, evidence-supported way to access this category of ingredient in the United States today. Microneedling creates controlled micro-channels in the skin that significantly improve the penetration and bioavailability of topically applied PDRN, allowing the molecule meaningful access to the dermal layers where its regenerative signaling can have an effect, without crossing into unapproved injectable territory.


This approach also allows for thoughtful combination with other actives. Research and clinical experience both support pairing PDRN with vitamin C and niacinamide, which enhance antioxidant protection and help preserve the extracellular matrix the PDRN is working to support. Biostimulatory injectables, a separate and FDA-approved category, work by triggering a controlled inflammatory and collagen-synthesis response through a different biochemical pathway entirely. For patients prioritizing legally available injectable collagen stimulation today, this category, combined with energy-based skin tightening modalities where appropriate, often represents the most clinically appropriate current option.


I walk through these comparisons with patients individually because the right answer genuinely depends on skin quality, treatment history, goals, and timeline — not on which option is currently generating the most social media attention.


Who Should Be Curious, and Who Should Wait


For patients who are simply curious about regenerative skincare ingredients and want to incorporate well-supported, legally available options into a topical routine, PDRN-containing serums and microneedling-delivered formulations are a reasonable and low-risk place to start, particularly when layered into a barrier-supportive, antioxidant-rich routine.


For patients specifically seeking the injectable version they encountered on social media, my honest clinical guidance is to wait. Not because the underlying science is illegitimate, but because the specific product category they are asking about does not currently have a legal, regulated pathway for injectable use in the United States, and pursuing it through unregulated channels — including medical tourism specifically for this treatment, or seeking out providers willing to inject unapproved products domestically — introduces real and avoidable risk for a benefit that has not yet been established to the standard we require for the other injectables I do offer.


This is, admittedly, not the answer that generates excitement. But it is the honest one, and I would rather tell a patient "this isn't appropriately available yet, and here is why" than perform a procedure I cannot stand behind with full confidence in its safety profile.


The Strategic Framework I Use With Patients


When a patient brings this topic to me, I walk through it in three tiers, the same way I approach most regenerative aesthetics conversations.


Foundational: a properly formulated topical routine, including barrier-supportive moisturizers, broad-spectrum sun protection, and antioxidant serums, remains the non-negotiable baseline regardless of which regenerative trend is currently popular. No injectable or device-based treatment, PDRN included, substitutes for this groundwork. Skin that is well-protected and well-hydrated responds better to every subsequent intervention.


Supportive: for patients wanting to explore PDRN's regenerative properties through a compliant delivery method, topical PDRN combined with microneedling is the appropriate current tier. This delivers genuine, research-supported benefit to skin texture, hydration, and the dermal environment, without venturing into unapproved injectable territory.


Corrective: for patients seeking more significant collagen stimulation, volume support, or skin tightening through injectable or energy-based methods, I direct them toward the FDA-approved biostimulatory, PRP-based, or energy-device options that are appropriate to their specific presentation — categories with a more established safety and efficacy record in this country today.


This framework will likely shift over time. Regulatory pathways evolve, and it is entirely possible that injectable PDRN or PN formulations receive U.S. approval in the coming years as more data accumulates. When and if that happens, I will incorporate it into the corrective tier with the same evidence-based scrutiny I apply to every other treatment in my practice. Until then, I would rather give patients an honest, current answer than a hopeful but inaccurate one.
