For a long time in the U.S., skincare followed a very simple belief: if something worked, you should feel it. Tingling meant it was active. Redness meant it was strong. Peeling meant it was effective. The more intense the reaction, the more confident people felt that they were “doing it right.”


And for a while, that approach seemed to make sense. Skin turned over faster. The texture changed quickly. Results felt visible.


What I’ve been seeing more and more over the last few years, though, is what happens after that phase. Patients don’t come in because they ignored their skin. They come in because they tried very hard to take care of it. They used good products. They followed routines they found online. They rotated acids, retinoids, vitamin C, and devices. They optimized everything.


And yet their skin is irritated. Reactive. Red when it shouldn’t be. Breaking out in ways that don’t follow a clear pattern. Sometimes thinner. Sometimes suddenly sensitive to products they used for years without a problem.


That’s usually the moment when people feel confused. They’re doing “everything right,” so why does their skin feel worse?


This is where the conversation has shifted.
